Peak House Youth Addiction Referrals & Intake

Peak House accepts referrals from parents, caregivers, healthcare providers, schools, and community organizations across British Columbia.

The referral can be completed electronically. Referring Counsellors are invited to forward completed referrals directly to Peak House by fax or email.

Referrals are forwarded to Vancouver Coastal Health, Centralized Addictions Intake Team (CAIT), and reviewed by the admission committee.

Intake Criteria

  • Resident of British Columbia
  • Willing to attend treatment (voluntary program)
  • Professional Support / Referring Counsellor
  • Housing plan
  • Mentally and physically able to safely participate in group living and group therapy

Intake Process

The Referral Process:

1.  Complete the referral package

2.  Forward your referral to Peak House and we will submit to vch on your behalf and/or submit to vch and Cc intake[at]peakhouse.ca

3.  Complete the Pre Intake Information forms as appropriate. This significantly helps us understand the best ways we can support and set up clients for success prior to entry.

4.  Referrals are reviewed weekly by the community screening committee

If you have an urgent need, please contact us directly. Often we’re able to prioritize high risk and treatment ready intakes.

The Intake Process:

Once a referral is accepted and a match in service assessed, our Intake Coordinator will contact the young person’s referring counselor to estimate an approximate intake date, forward the Pre-Intake information requests to the client, caregiver/parent, referring agent if we have not already received it, and arrange to speak with the youth directly. A pre-intake meeting with be set either in person, over video or the phone. The pre-intake meeting is a chance for youth to get more information about the program and for us to get to know more about the youth before they come in.
